Duties & Responsibilities
Programmatic
- Partners with ethnic community and/or faith-based organizations and providers to develop, deliver, and/or promote culturally and linguistically appropriate services for the population served.
- Gives priority to individuals that meet the priority criteria and with urgent needs or emergency situations.
- Coordinates and facilitates eligibility screening for individuals to be served.
- Coordinates and facilitates intakes for individuals to be served, including assessment, case management, and emergency assistance.
- Coordinates and facilitates emergency assistance services for individuals served that reduce the risk of homelessness and food insecurity.
- Assists in the formulation and implementation of policies and procedures as per regulatory requirements and makes revisions as needed.
- Ensures that all files include documentation in compliance with funders and Catholic Charities policies and procedures requirements.
- Adheres to the dress code and wears appropriate clothing and follows safety protocols per OSHA and Catholic Charities when conducting job functions.
Human Resource Management
- Provides supervision, direction, and support to program staff.
- Ensures orientation and training is provided to staff according to regulatory body requirements.
- Schedules and conducts weekly staff meetings.
- Ensures that staff receives required training and professional development within the specified time frames.
- Provides on-going supervision to each staff under the incumbent’s supervision.
- Conducts performance evaluations and/or improvement plans for all staff directly under his/her direct supervision.
Risk Management and Quality Assurance
- Collects outcome information and submits timely and accurate reports for the program as indicated in the contract.
- Completes and submits timely and accurate Significant Incident Reports, CIP Reporting and any other required documentation as per Catholic Charities guidelines, state licensing requirements, funder requirements and COA Standards.
- Responsible for following up on all incidents reported timely and accurately.
- Participates monthly in the fire drill.
- Annually assists in completing and submitting Tactical Plans.
- Annually assists in the distribution, collection, and aggregation of Consumer Satisfaction Survey.
- Participates in the Performance Quality Improvement process.
- Monthly, Quarterly, and Annually track services for the completion of Quality Assurance Reports.
- Monitors the compliance of safety and health precautions and monitors staff compliance according to all governing bodies’ policies and procedures.
Screening & Assessment
- Screen of potential consumers within 7 days of referral.
- Maintains wait list for consumers unable to be served immediately and initiate services based on level of need.
- Complete strength based comprehensive biopsychosocial assessment that includes the client’s medical, psychological, and social needs.
- Comprehensive biopsychosocial assessments should be strength-based, trauma-informed, and culturally and linguistically responsive and utilized to determine individual’s trauma induced impairments or mental health needs.
- Utilizes screening instruments for trauma, depression, anxiety, suicidality, and/or substance abuse as applicable.
- Completes assessments within 30 days of intake.
Wellness Service Planning & Monitoring
- Develops a service plan within 30 days of intake.
- Develops an Aftercare Plan within 30 days of intake.
- Develops a service plan with outcomes aligned with client-identified goals that identify specific areas of vulnerability that merit individual’s enrollment in the program, including but not limited to mental health services to be received and case management self-sufficiency goals.
- Develops family-focused service plans that include services for eligible family members that include family-strengthening interventions and conflict transformation.
- Creates service plans based on the assessment that includes client-identified agreed goals and objectives; specific, measurable, realistic, and time-bound goals; and services and support to be provided.
- Reviews and updates each consumer’s service plan quarterly.
Therapeutic and Educational Interventions
- Utilizes specific and developmentally appropriate interventions for vulnerable subgroups such as women, LGBTQI+ individuals, children, and youth.
- Offers mental health services such as individual and group therapy, to trauma-affected newcomers, including vulnerable or marginalized groups such as survivors of domestic violence (DV), intimate partner violence (IPV) and gender-based violence (GBV); children and adolescents and (LGBTQI+) individuals who may be experiencing social, mental, emotional, and familial challenges.
- Engages with consumers and families by providing weekly or biweekly therapy.
- Provides individual and group therapy to individuals and their families, including family strengthening interventions, as indicated.
- Enters weekly progress notes into the electronic record.
Operational Job Function
- Facilitates weekly interdisciplinary staff meetings.
- Reviews weekly with the team caseload and consumer progress.
- Participates in case record review monthly.
- Maintains compliance to all Agency, Government, Funding Sources, COA and Catholic Charities.
Case Closing
- Ensures case closing begins at intake and culminates with the consumer completing the agreed upon service goals.
- Issues closeout letters information individuals served of the successful achievement of their self-identified wellness goals and connecting them with relevant ORR and community resources.
- Upon closing a case all collaborating service providers are notified if applicable.
- Follow-up by phone post discharge with the permission of the consumer.
Additional Duties
- Incumbent will comply with all policies, procedures, and requirements necessary to perform the functions of this position.
- Completes other tasks as requested by supervisor.
Physical Demands
- Ability to travel throughout Miami-Dade and Broward counties for meeting/training locations.
- The work is usually sedentary, but may require long periods of standing, walking and /or bending and some light lifting.
Educational & Experiential Requirements:
- Master’s degree in Social Work
- Licensed or license eligible mental health professional.
- Bilingual in English/Spanish required.
