Position Overview:
JFS is looking for a compassionate, mission-driven individual to join the Refugee Resettlement team as the Health Navigator. In this role, you have the opportunity to use your organizational skills and public health experience to make a meaningful difference in your community and help change lives across San Diego. The Health Navigator plays an integral role on the team, working along with other Health Navigation staff. The role requires strong attention to detail, data entry skills, and the ability to complete regular reports and submit them in a timely manner; this work is done in close coordination with the Program Manager and other Resettlement staff. This position reports directly to the Preferred Communities Program Manager and works closely with the Preferred Communities team and the rest of the Refugee Resettlement team.
Responsibilities:
- Coordinate refugee health screenings for all new arrivals through the Refugee Health Assessment Program (RHAP)
- Coordinate client’s transportation to scheduled medical appointments, including urgent/emergency or specialty medical care health services
- Assess and coordinate referrals to mental health providers as needed
- Assist patient to enroll in a Medi-Cal managed care plan and select a primary care physician
- Provide assistance with telehealth appointments through education and/or access to appropriate technology
- Conduct community outreach and presentations to develop a system of support on behalf of eligible populations
- Participate in collaborative and work group meetings related to health care services
- Manage data and documentation per program guidelines
Skills/Experience/Abilities That Are a Must-Have:
- Familiarity with US healthcare system and healthcare programs for low-income families strongly preferred
- 2 years of experience with health education or working in the medical/health related field
- Experience working with refugee and/or immigrant populations strongly preferred
- Self-starter with excellent problem-solving skills, proven ability to multi-task, prioritize duties, and manage time effectively
- Strong relationship building, diplomacy, and networking skills; ability to effectively build internal and external relationships
- Demonstrated success working and communicating effectively in a multi-cultural environment
- Experience in a nonprofit setting, preferred
- Advanced use of MS Office programs, including Outlook, Excel, and Word
- Ability to manage multiple priorities in a fast-paced environment
- Excellent communication skills and customer relationship skills
Skills/Abilities We’d Like You to Have:
- Fluency in second language: Arabic, Dari, Haitian Creole, Swahili, French or other
