The Outreach and referral Processing Coordinator will represent the company and work with the Chief of Operations to support outreach efforts in the Central Mass area. They will also be responsible for processing all potential referrals and supervising volunteers.
Duties & Responsibilities:
- Conduct screening of potential clients
- Conduct the initial visit to the client's home and check the qualified settings
- Complete paperwork for clients and caregivers during the initial visit
- Maintain referral tracking system on a real-time basis
- Nurture new and old relationships with collaborative partners
- Prepare accurate records and reports
- Provide regular updates to the Operations Manager regarding referral status
- Ability to work occasional nights and weekends
- Other job duties as assigned

- Fluency in Spanish preferred
Education & Experience:
- Associate’s degree required
- 2 years of outreach and marketing experience
- Knowledge of Human Services required
- Knowledge of Health Insurance plans preferred Other Requirements:
- Valid driver’s license Physical Demands:
- Ability to be physically mobile (stand, walk, reach, climb, bend) and also the ability to sit for prolonged periods throughout the day/work hours
- Manual dexterity sufficient to operate keyboard, copier, telephone, and the ability to write and/or type
- Must be able to occasionally lift and/or move up to 25 pounds
- Frequent driving to client appointments, offsite meetings, conferences, etc.
