STATEMENT OF THE JOB:
The Team Supervisor oversees a team and has the responsibility to focus the team"s activities as it pertains to housing placements. The Team Supervisor will direct and oversee the team"s efforts to help clients navigate their housing applications from start to finish.
The Team Supervisor is also responsible for ensuring that team"s activities are person centered, culturally competent and linguistically capable. S/he will problem solve any obstacles that will hinder a client"s housing placement.
* Oversee the proper follow up of all clients housing placement applications.
* Monitor outreach and engagement activities of the team, which will support hosing placements.
* Regularly assesses the team"s caseload needs and goals and clearly identify reported progress in meeting goals by individual health home members as well as compliance with recommended treatments.
* Conduct regular case reviews and provides strategies for housing placements.
* Conduct Psychosocial Assessments and complete housing applications as necessary.
* Lead team meeting to assure that communication is fostered between team members.
* Supervise the quality of contacts by staff with clients, housing programs and treatment provide.
* Conduct workshops, trainings and regular supervision for the team staff.
* Evaluate staff performance and develop performance improvement plan, as needed.
* Provide direct member coverage as needed.
* Cooperate with any and all investigations conducted by the Agency, funding sources and any other authorized agencies/entities.
* Reports to Integrated Health & Wellness Services Administration and/or Agency Administration issues that may have a negative impact on the reputation of the Agency, client and/or staff welfare or any corporate compliance issues.
* Performs other related duties as requested or assigned by agency management.
QUALIFICATIONS:
* Master"s Degree in Social Work, Nursing, Public Health or other related social service or health profession
* Must have a minimum of (1) year experience in similar setting or situation.
* Must have two years of prior supervisory experience in the mental health field.
* Operate office equipment as necessary, i.e. calculator, copier, computer, fax, etc.
* Requires a combination of skills in the following areas; operational oversight and evaluation of program services and staff, personnel management, public relations, governmental relations, leadership development and team building skills.
* Comfortable with public speaking, lobbying and advocacy.
* Bilingual speaking a plus.
* Knowledge of HRA 2010E and NYC Housing Voucher.
* Able to utilize technology conferencing tools including audio, video and /or web deployed solutions and accountable for hand-held devices (I Phone, Blackberry, I Pad, Tablets, Laptops, etc.)
* Frequent sitting to write reports and when meeting with clients.
* Ability to read printed materials and computer screens. Ability to work flexible hours and days - including weekends/evenings/holidays according to needs of a 24/7 program.
* Ability to travel in the community and use public transportation.

Pay: $55,000.00 - $61,600.00 per year
