POSITION SUMMARY
The Community Outreach Coordinator supports the CHS mission by seeking, creating and maintaining connections with
organizations and businesses within the community to partner with CHS in providing opportunities for individuals
supported in the Community Support Services program.
ESSENTIAL DUTIES AND RESPONSIBILITIES
As the Community Outreach Coordinator, you will:
- Publicly represent CHS by building relationships within the community; developing opportunities to partner with community businesses and services in activities and projects; serving within the community as a representative of the CHS organization and mission; representing the organization at public, social and business events, including participating as a member of the Chamber of Commerce; advocating for inclusion; and supporting accessibility, and other rights of the Individuals we support
- Promote awareness of Community Support Services programs by giving presentations in the community; writing articles for local publications to promote CHS, highlight our work, and reflect our mission and values; engaging the community through email and social media; and participating in interviews, and other media opportunities, all in collaboration with the Outreach and Communications department
- Lead community participation and integration by communicating with and supporting the choices of Individuals in the types of community activities they are most interested in, and recording the same for future plans; leveraging community knowledge, partnerships and other relationships to create unique opportunities for Individuals’ participation in community events and activities to further growth in personal and ISP identified goals; planning and preparing for community outings, ensuring that all necessary tools, including transportation and supplies, are available and ready to maximize the success of activities which connect Individuals with their community and with one another.; working overtime and/or outside of regular program hours, when necessary, to lead, supervise or support after-hours activities; developing and maintaining an activity calendar to support variety, and an efficient and orderly utilization of time and other resources; and communicating with Individuals and staff in after-action reviews to identify and remove barriers to future success
- Support Individuals in achieving greater outcomes by designing individualized programs and implementation strategies; consulting with caregivers, and conducting professional observation and assessments; advocating for Individuals by locating and accessing services and supports; engaging in continuous learning about the issues facing our industry and the Individuals we support, on a local, state, and national level; and proactively providing direct instructions in the implementation of specialized programs to enhance self-direction, independent living skills, community integration, social, leisure and recreational skills
- Promote interdepartmental harmony by working with Community Living (CL), Family & Community Supports (FCS), Employment Services (ES)and other ID/D related service lines to develop knowledge, increase cohesion, and support positive change in policies, procedures, and other departmental and interdepartmental practices; and by participating in cross-departmental committees or projects when possible

COMPETENCIES

- Capable of learning proprietary systems quickly, experience in ATS, HRIS/EIS systems a plus
- Capable of qualifying, testing, commissioning and bonding as a Registered Notary Public, if requested

QUALIFICATIONS
REQUIRED EDUCATION and EXPERIENCE
- Bachelor’s degree (BA or BS)
- Experience working in the field of intellectual or developmental disabilities preferred.
- Experience working with non-profit organizations preferred.
REQUIRED MISCELLANEOUS QUALIFICATIONS
- Valid driver’s license
- Reliable, licensed, and insured vehicle
- Reliable form of communication
PHYSICAL REQUIREMENTS
- Ability and strength to position/lift/transfer greater than 50 lbs. (an adult human being) regularly, including the ability to safely assist an adult human being to the ground to perform CPR
- Continuous ability to push and pull 100-125 lbs. of pressure consistently over extended periods as per requirements of performing CPR, as defined by the American Heart Association
- Continuous ability to kneel on hard or other surfaces for extended periods, as per requirements of performing CPR
- Lift, carry, squat, bend, move, push or pull, sit or stand repetitively and for long intermittent periods
- Type on a keyboard for extended periods, and view a computer or other electronic screen for intermittent periods
- Walk and/or stand for extended periods
- Drive distances of 200 + miles, as needed
