SNAP is seeking a Singles Homeless Coordinated Assessment (SHCA) Lead to join our Essential Connections Core. This vital role provides a single point of entry for homeless single individuals and couples without children, ensuring they receive the services and support they need to rebuild their lives.

Starting salary for this position is $23.82-$26.69 per hour DOE

Full salary range is $21.26-$33.55 per hour

Position Summary: Helps to oversee and administer the SHCA program within Homeless Services. Conducts assessments to homeless individuals, trains partner agencies on the assessment tool, provides outreach, ensures data integrity and program evaluation. Develops and implements a comprehensive plan of assessment, including planning, service delivery, short-term follow-up, and support. Serves homeless clients as a case manager.

Principle Duties and Responsibilities:

Serves as the project lead for the SHCA program, which provides a single point of entry to the homeless response system for homeless single individuals and couples without children.
Participates in activities such as coalitions, committees and task force meetings to plan and develop resources to address community needs. Represents SNAP on appropriate community committees and boards, and with the media as approved and allowed by funding sources.
Maintains and builds network of SHCA satellite assessment partner agencies. Conducts training and ensures consistency on the SHCA assessment tool with partner agencies.
Participates in local and statewide initiatives to end homelessness within the singles and chronic system, including case conferencing with community partners.
Empowers participants to achieve economic and personal independence and self-sufficiency by providing case assessment management services to homeless individuals.
Ensure participant files are accurate and updated. Maintains required case and program records as required in an accurate and timely manner.
Ensures data entry into multiple databases is entered accurately and in a timely manner.
Conducts intakes and assessments to determine eligibility for homeless services.
Runs reports and works with the MIS team to create new reports as needed. Assists with monitoring grants and budgets with management support and assistance.
Provides information and referral services within the agency and to external partners and satellite providers as requested.
Conducts outreach to complete assessments with homeless individuals on a regular basis.
Works collaboratively with other team members and providers to assure continuity of services.
Provides individual and system advocacy where appropriate within the guidelines of SNAP’s Advocacy policy.
Collaborates with City of Spokane CHHS staff to ensure data integrity and communication with partner agencies.

Attends staff meetings and training as scheduled.

Completes assigned projects and performs related duties as assigned or as the situation dictates.
Provides excellent customer service at all times and helps to grow the customer service culture within the agency.

Qualifications

Requirements:

Education and Experience

Required: Minimum of three year’s case management experience or related field, or a combination of education and/or experience totaling 6 years.
Preferred: BA in social services with five year’s case management experience or related field. Spanish, Russian, Vietnamese and/or Marshallese bilingual skills are preferred.

Knowledge, Skills, and Abilities

Ability to empathize with and advocate for individuals with diverse backgrounds and needs, including those from low-income households
Ability to work with vulnerable populations such as those re-entering the community from institutions such as jail, prison or mental health facilities.

Demonstrated communication, life management and motivational skills to promote self-sufficiency rather than supporting dependence
Ability to work with various assessment and planning tools, including family development model
Knowledge of local resources with the ability to network and link appropriate services together to meet individual and family needs

Exceptional interpersonal skills with emphasis on effectively working in a team environment and with a diverse population

Proficient in computer operations in MS Office, including word processing, spreadsheet, and database (Salesforce, CMIS); aptitude and ability to learn related technical skills

Ability to communicate confidently, safely, and effectively with people in crisis situations

Training and Certifications

Bloodborne pathogens training (annually)
Valid drivers license, satisfactory driving record, personal vehicle and insurance for Agency business (mileage reimbursed)

Work Environment/Physical Activities:

Some work is sedentary, performed in a typical office environment. Incumbent will make visits to participants’ homes or living sites, and may encounter unsanitary conditions and difficult people. Work does require local travel to offices, residents, and other buildings. May require lifting up to 20 pounds.
