SUMMARY
Directs daily operations of assigned group home(s), overseeing individuals with intellectual/developmental disabilities as well as staff serving them. Ensures assigned home(s) comply with Ohio Department of Developmental Disabilities (DODD) and Medicaid Certification standards and regulations as an agency provider of waiver services. Inspires and leads staff with positivity, involving them in the organization's mission, vision, values, and strategic goals. Assist your identified supervisor(s) with program duties and ensure the success of individuals in accordance with their Individual Service Plan (ISP) and person-centered plans.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Complete all billing in an accurate and timely manner for services performed by the deadlines set by the Administration Office. This includes timely and accurate documentation of individual’s and their services.
Responsible for the administrative supervision of home site including but not limited to ordering supplies, scheduling events, managing facility needs, NMT and coordinating tours and care schedules.
Maintain a positive work atmosphere by behaving and communicating in a professional manner that gets along with individuals, co-workers, community partners, and supervisors. This also includes guardians/parents, caregivers, providers and any other agencies you may have contact with.
Ensures consumers’ needs are met as outlined in their Individual Service Plans (ISPs). Monitors and remedies Quality Assurance issues in assigned homes.
Performs regular visits to each home to review cleanliness, quality of care, and accuracy of documentation, files, physician’s orders, financial records, etc.
Communicates with each individual’s team, including guardians (if applicable) and/or County Board of Developmental Disabilities.
Participate in the annual Individual planning/meeting process for those residing in the home and provides input as a member of their team.
Develop and implement programming designed to increase daily living skills, personal and social adjustment, and behaviors and skills.
Provide case management services to individuals, including identifying and coordinating community resources and services.
Develop and instruct downtime and independent living activities.
Creates schedules to meet individuals’ needs while balancing the self-care needs of staff.
Ensures adequate, safe staffing for programs as well as assigned homes. May reassign staff in collaboration with supervisor(s) and other House Leads based on emergent needs to meet the demands of other services, locations, and/or programs under ERLI.
Coach and assist staff with developing, designing, implementing and revising individual service plans including long and short-term goals.
Ensure all required documentation is completed by the individual’s plan, ERLI policies, procedures and oversight agencies is completed and filed as appropriate.
Participate in department Program Evaluation and Outcome Measurement activities.
Facilitate behavioral supports specific to individual needs.
Attend agency and department meetings.
Recommend and implement program improvements.
Assist with tracking required training including but not limited to CPR, First Aid, and Driving Credentials (when necessary) and continue to encourage professional development (membership in organizations like ORA or NJSST, workshops, and seminars). Minimum requirement of 15 hours of professional development per year outside of in-service dates. Requirements can be met through DODD online and the agency offered training.
Ensure and advocate for the rights of the individual and recognizes and respects individual differences and preferences.
Ensure Unusual and Major Unusual Incidents are documented and reported according to Ohio Revised Code regulations and the health and safety of the participants served is always a priority. Document Unusual Incidents/Major Unusual Incidents (UI/MUI) in case management system. Notification to Supervisor, family, Support Administrator, and others identified required within compliance requirements.
Cooperates with County Board of Developmental Disabilities’ Investigative Agent on the investigation of Major Unusual Incidents (MUIs), including any allegations of abuse/neglect, related to assigned homes.
Maintain caseload documentation of case notes, ISP revision dates, documentation, recommendations for transition to next opportunities etc. and keep supervision up-to-date regarding individual progress. Caseloads are built based on acuity, agency needs, and individual’s skills and interests.
Communicate regularly with individual’s Support Administrator through phone calls, email and meetings; keeping the Support Administrator apprised of the individual’s activities and progress in the program. Ensure all communications are documented in the case management system.
Communicate effectively to Supervisor and Support Administrator when span dates are approaching expiration. This should be communicated ideally 60 days, minimally 30 days prior to expiration to avoid no payments for providing services.
Maintain accurate financial records in accordance with compliance and accreditation requirements for individual’s served in the home.
Maintain accurate documentation on medication management, and distribution of medication for individuals served.
Use modeling techniques to instruct individuals and groups in proper work methods and positive work behaviors and how to improve work quantity and quality.
Write monthly reports regarding an individual’s progress and ensure to send to individual’s team as outlined in their plan.
Act as liaison to referring Support Administrator, family, guardians, and other individuals who are actively involved in the individual’s life.
Provide transportation to individuals for appointments, shopping, outings, and so forth in an agency vehicle.
Complete and maintain CPR/First Aid and Delegated Nurse training during entire duration of employment.
Emergency calls/messages may be outside of scheduled hours in the home(s). While many emergencies can be addressed by phone/video or other staff during off hours, occasionally, the House Lead may need to report in-person to assigned homes to assist in these situations.
Participates in a regular rotation of being the designated weekend on-call contact for homes and must be accessible during these times.
Must be willing to travel and work shifts at other ERLI locations and/or departments to assist in providing services to individuals as needed.
Ensure staff maintains compliance with all training requirements.
Train staff in required and other programmatic areas.
Maintain appropriate staff coverage and ratios.
Plan and direct work assignments and caseloads for all staff.
Must be willing to step in and provide NMT services based on the agency’s need.
Evaluate staff performance, including providing feedback to and disciplining staff and addressing complaints and resolving problems in collaboration with supervisor.
Responsible for following agency safety policies and maintaining a safe work environment.
Maintain regular attendance per ERLI policy.
Will provide direct care to individuals, including but not limited to supervising daily activities, ensuring safety and security, and assisting with personal hygiene and self-care tasks.
Will maintain a therapeutic environment by utilizing positive behavior management techniques, providing consistent and clear expectations, and promoting a sense of community among clients.
Maintain a positive work atmosphere by behaving and communicating in a manner that gets along with customers, vendors, individuals, co-workers, and supervisors.
Must be willing to travel and work shifts at other ERLI locations and/or departments to assist in providing services to individuals as needed.
Other duties as assigned.
SUPERVISION
Responsible for overseeing staff working in assigned supportive living homes. Directly supervises Direct Support Professionals (DSPs) in each assigned home(s). Guides and trains staff regarding compliance matters.

Program specific needs will be identified upon assignment.
