Job Overview:
We are seeking an experienced and detail-oriented Pharmacist with 2-3 years of experience to ensure safe and effective medication use, collaborate with healthcare teams, and deliver excellent patient care.
Key Responsibilities:
- Medication Management: Ensure safe and accurate dispensing, monitor drug interactions, and optimize medication therapy.
- Collaboration with Healthcare Team: Work with doctors, nurses, and other professionals to improve patient care and treatment plans.
- Compounding & Special Needs: Prepare customized medications, including sterile formulations, for patient-specific needs.
- Patient Safety: Review prescriptions for appropriateness and provide recommendations to prevent adverse effects.
- Patient Counseling: Educate patients on proper medication use, side effects, and adherence.
- Adverse Drug Reaction Monitoring: Identify and report adverse drug reactions and adjust treatment plans as needed.
- Dispensing Medication: Accurately dispense and label medications, ensuring proper dosage and instructions.
- Clinical Pharmacy Services: Provide medication reviews, therapeutic monitoring, and support in clinical rounds.
- Inventory Management: Maintain medication stock levels, monitor expiration dates, and ensure regulatory compliance.
