Description

Skyland Trail, a nonprofit mental health residential treatment program for adults and adolescents in Atlanta, is seeking a Independence Living/Transitional Coach

Ensure that each client in their caseload is sufficiently supported by the Skyland Trail program services from the client’s entry into the case management program until discharge.
Be responsible for well-planned and coordinated recovery-based services within the case management community program.

Responsibilities:

Monitor clients’ Activities of Daily Living (ADLs), house upkeep, and other pragmatic needs within the community. Assist, teach, and model daily living skills, healthy boundaries, and communication skills.
Orient and assist with logistics for client’s entering Transitional Housing. Conduct first session, when Transitional housing paperwork is signed, at the apartment to immediately address questions or concerns about the facilities.
Develop recovery plans and discharge instructions for case management clients following Joint Commission standards and agency guidelines and revise as necessary, but at least every 3 months. Continue on-going updates to client charts as they relate to assessments, psychosocial history and clients’ well-being and document in the Electronic Health Record (EHR).
Document changes to client’s appointments, as well as employment/educational goals and attendance.
Conduct personal one-on-one visits to clients’ apartments according to client needs at least one to two times weekly. Utilize case management skills, such as, advocacy, monitoring, and discharge planning to link clients to resources and services. Make referrals to community and internal resources and document, including physicians as appropriate.
Encourage client participation in community supportive and social functions. Monitor client attendance of community events as appropriate to the level system.
Complete treatment plans for all residents which are updated every 90 days.
Monitor client medication adherence by discussing medication checks with the Transitional nurse. Prompt clients to order refills and assist with communication to pharmacy and prescribing physician if necessary.
Provide updates for clients’ families at least once per month.
Reach out to coordinate care for outpatient providers and contact outpatient providers with clinical concerns and feedback. Participate in bi-weekly Transitional Housing phone call to coordinate care with clients in intensive services.
Complete urine drug screens weekly with clients who are no longer enrolled in intensive services at Skyland Trail.
Notify Manager of Transitional Housing and psychiatrist of any changes in clients’ mental status/behavior and maintain open communication among client, counselor, director, psychiatrist and family. Alert primary counselor or community therapist if client misses appointments (no face to face contact) for more than 2 weeks.
Create appropriate wellness plans with clients as needed. Report the final wellness plan to the individual’s recovery team and supervisor, document in the EHR.
Attend weekly recovery community meetings and provide pertinent updates on client progress.
Attend supervision with Medical Director to review client cases, clinical concerns and training. Attend weekly scheduled supervision with Transitional Program Manager for administrative supervision, case conference, and clinical direction.
Participate in facility quality improvement projects as needed. Participate in weekly programming meeting focused on improving the Transitional program.
Facilitate tours of the Transitional campus, orienting clients to the rules and responsibilities at Transitional Housing.
Facilitate Day Treatment groups, as assigned.
Complete monthly inspection of transitional housing apartments to identify maintenance issues and client care concerns. Submit work orders for any facilities issues.

Qualifications

Competencies/Qualifications:

Bachelor's Degree OR 2 years previous experience required.

Formulate and update measurable treatment goals and objectives
Recognize the importance of family and support systems in the treatment process
Participate in continuing professional development
Crisis prevention, intervention, and management skills
Able to complete documentation in electronic health record in a timely manner
Knowledge of emergency procedure for fire, disaster, and unplanned incidents
Knowledge of CPR and First Aid Procedure and involvement in provided training at Skyland Trail
Utilizes and adheres to HIPAA standards and Skyland Trail Confidentiality Policy
