JOB SUMMARY: This position is responsible for the coordination as well as delivery of clinical and prevention services designed to meet the wellness and recovery needs of the youth. Depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumer’s family members both in the program and in the school setting.
ESSENTIAL FUNCTIONS
- Facilitate group/individual sessions for youth and/or parents
- Provide knowledge regarding substance abuse (tobacco and other drugs)
- Develop critical life skills, social skills and coping abilities
- Access prevention education needs of students and implement curricula as indicated during sessions
- Participate in program evaluation by administering pre/post test
- Document content of group and individual sessions, noting attendance and participation
- Attend annual trainings and staff meetings
- Conduct initial and ongoing diagnostic assessments involving the collection, analysis, and recording of significant bio-psychosocial information.
- Youth Group Facilitator works in schools and communities to facilitate substance abuse prevention services based on Botvin Life Skills curriculum.
- All documentation should be maintained in the systems(Millennium/POMS), the agency’s electronic health record software within 24 hours of service delivery, and should include: the assessment, diagnosis form, treatment plan, progress notes, collateral contacts, and termination summary.
- Case management services are to be provided as needed and may include preparation of reports for external entities, community linkages, home based counseling, assistance in obtaining benefits and entitlements, referral and other client advocacy services.
Supervision: Supervised by Program Supervisor, the Youth Group Facilitator works in program, schools and communities to facilitate substance abuse prevention services based on Botvin Life Skills curriculum.
OTHER DUTIES
- Participate in the Quality Improvement process and specific quality initiatives for the program.
- Interface with school, medical, vocational, residential and other program resources as necessary.
- Work as part of a team to provide services to youth outside of primary caseload as needed.
- Performs all duties in a professional and ethical manner consistent with agency, licensing, COA, and professional standards.
- Completes the initial evaluation (CASI) and referral if appropriate.
- Provide crisis intervention as required. (New Choices/Prevention Plus).
- Provides school prevention education services. (Prevention Plus).
- Communicates observations and information about the youth and children to the program supervisor and treatment team. (New Choices/Prevention Plus).
- Collects urine specimens for urinalysis (New Choices).
- Leads educational groups (New Choices/Prevention Plus).
- Completes all required documentation in Millennium and POM (Prevention Outcome Measurement (New Choices/Prevention Plus).
- Other related duties as assigned.
