Description

The Housing Support Coordinator (Case Advocate) is responsible for providing comprehensive case management and advocacy for clients involved in three key housing programs: Crisis Housing, Pathways (Rapid Re-Housing), and Homestead. This role includes assessing client needs, developing individualized service plans, and connecting clients to necessary resources and services to promote housing stability and self-sufficiency.

ESSENTIAL FUNCTIONS

Program Coordination and Case Management:
- Manage a caseload of clients in both Crisis Housing (Bridge House), Pathways (Rapid Re-Housing), and Homestead programs.
- Conduct initial assessments to determine client needs, eligibility, and appropriate program placement.
- Develop and implement individualized service plans in collaboration with clients, focusing on housing stability, financial independence, and personal goals.
- Provide ongoing case management support, including regular check-ins, progress monitoring, and adjustments to service plans as needed.
Client Advocacy and Support:
- Advocate on behalf of clients to secure necessary resources, such as housing, employment, education, and healthcare.
- Assist clients in navigating housing applications, lease agreements, and tenant rights.
- Mediate and resolve any conflicts or issues between clients and landlords or other service providers.
Resource Coordination and Referrals:
- Identify and establish partnerships with community organizations, landlords, government agencies, and service providers to enhance the support network for clients.
- Connect clients with appropriate resources, including mental health services, substance abuse treatment, job training programs, and financial assistance.
- Maintain a comprehensive directory of community resources and referral options.
- Instead: Unitize established community and state resource and referral options, conduct research for additional options based off client barriers.
Program Monitoring and Reporting:
- Track and document client progress, service utilization, and program outcomes using case management software.
- Prepare regular reports on program performance, client outcomes, and areas for improvement.
- Ensure compliance with program guidelines, funding requirements, and confidentiality standards.
- Complete necessary entry, annual, and exit assessments for data collection.
Community Outreach and Education:
- Conduct outreach activities to raise awareness of housing programs and services within the community.
- Facilitate workshops and educational sessions for clients on topics such as budgeting, tenant rights, and job readiness.
- Represent the organization at community events, meetings, and coalitions to promote housing stability initiatives.
Administrative Duties:
- Maintain accurate and up-to-date case files, documentation, and client records.
- Assist Housing Program Manager in the preparation and submission of grant applications and reports.
- Participate in regular team meetings, training sessions, and professional development opportunities

Qualifications

Associate's degree in social work, human services, or a related field (or 2 years minimum relevant experience in case management, housing services, or a similar role.)
Basic knowledge of housing programs, tenant rights, and community resources.
Instead: Basic knowledge of state and federal resources, tenant rights, and crisis intervention.
Strong organizational, communication, and problem-solving skills. Ability to navigate challenging situations with resilience and establish clear boundaries.
Ability to work independently and collaboratively with diverse populations and programs team.
Proficiency in Microsoft Office Suite and case management software.
Valid driver's license and reliable transportation.
