Description

SCOPE OF RESPONSIBLITIES:

The Community Service worker is responsible for facilitating the provision of treatment and supportive services for clients who are transitioning into the community or home from residential care. The position also is responsible for providing specific services to the child’s family. The Community Service Specialist will be responsible for clients admitted to a residential center and targeted for community living or reunification. The Community Services Specialist may have responsibilities with clients in residential sites and in the community. This employee will ensure compliance with safe work practices and maintain safe environments in compliance with established agency standards, licensing regulations and generally accepted practices.

TASKS/DUTIES/RESPONSIBILITIES

Prepare client for transition into their own apartment.
Responsible for Apartment search.
Responsible for establishing residency
Responsible for opening checking and savings accounts.
Maintain contact as required by clients treatment plan.
Assist client in the following support services:
- Budgeting
- Recording of productivity
- Shopping
- Apartment maintenance
- Socialization opportunities
Provide supportive counseling as required by client’s treatment plan
Attend all service debriefings, progress meetings, or treatment plan meetings
Responsible for written documentation of all service activities.
Provide training and supervision required to prepare clients for discharge home
Provide home visit oversight specific to gauging clients ability to adjust to the home environment
Provide plan identified services after discharge to a 45 day permit
Daily contact for 30 days (4 face to face 3 by phone weekly)
Provide written progress note for each service activity
Provide supportive counseling as identified in client discharge plan
Provide transportation as required
Provide all support services as identified in the client’s treatment plan and care plan
Responsible for written documentation of all services activity.
Employees are required to use their personal vehicles in certain circumstances and as directed in the performance of their job duties.
Other duties as assigned.

Qualifications

Bachelor Degree in Human Services field preferred.
High School or GED.
Valid Driver’s License and reliable transportation required.
Use of vehicle to transport clients required.
High level of verbal and written communication skills required.

Physical Capacity Requirements

Driving
Frequently operates a motor vehicle both locally and/or long distances during daylight hours and/or after dark.

Lifting
Frequently lifts supplies, equipment, luggage, groceries, files, records, etc. up to 25 pounds.

Standing and Sitting
50% of the time is standing and walking.

Flexibility
Frequently requires stretching, bending, stooping and reaching.

Pulling & Pushing
Frequently engages in general sweeping, moping and movement of equipment or material.

Crises Management
Occasionally intervenes with clients engaged in physically assaultive, self-harmful behavior or serious property destruction to de-escalate and stabilize behavior. Intervention may require brief physical restraint.

Training
Occasionally participants in training for Physical Crisis Intervention, First Aid, and CPR. All of these require some physical activity: bending, kneeling, stooping, use of arms and legs.

Vision
Constantly requires close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.

Talking & Hearing
Constantly requires the ability to communicate verbally both in person and via telephone.

Hands & Fingers
Constantly requires ability to write, enter data via keyboard, handle tools, etc.
