Location: Cape Girardeau, MO

Join our team as a Care Manager at LifeHouse, where you will provide comprehensive case management to pregnant women experiencing homelessness, as well as aftercare clients, guiding them toward self-sufficiency. As a Care Manager, you will work closely with a multi-disciplinary team to develop personalized care plans, ensure clients' adherence to program expectations, and advocate for their needs within the community. You will engage with diverse populations, providing services such as transportation assistance, case management, and referrals to vital resources. With a strengths-based approach and a trauma-informed perspective, you'll help clients achieve their goals in a safe and supportive setting.
What You’ll Do:
- Provide comprehensive case management to residents and aftercare clients for the purpose of self-sufficiency.
- Attending community networking meetings by representing the agency.
- Create and update individualized care plans for residents and clients per the requirements and needs of the individual.
- Assist residents with transportation.
- Conduct intakes and face-to-face interviews and orient new residents.
- Provide case management with each client in combination with phone and face-to-face visits in varied settings, including the client's home in the aftercare program.
- Ensure residents complete chores, pass room checks, and upholding their commitment to the agreements and expectations of the program. This includes but is not limited to coordinating and monitoring class schedules, utilizing corrective action plans, administering drug screenings, etc.
- Complete written and data-based documentation (i.e., internal documentation case notes, housing plan, etc.)
- Advocate and interact with other services providers, parent educators, housing agencies, churches, and community organizations on behalf of participants, as well as facilitate referrals.
- Performs other related duties as assigned.

Minimum Qualifications
Required:
- Bachelor’s degree in social work (BSW) or equivalent human service degree.
- This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.
Preferred:
- Experience in case management and homeless/at-risk of homelessness populations.
- Knowledge of social services and referral sources for families.
- Motivational interviewing skills.
