Full Life Care, a Transforming Age Affiliate, is seeking a Care Coordinator to develop Health Action Plans and support healthcare management for home service clients. In this role, you'll provide care management, coordination, health promotion, education, and more, while collaborating with interdisciplinary teams to ensure quality care. Our Care Coordinators:
- Engage beneficiaries monthly through Health Action Plans, maintaining a caseload of 50-55 for quality service.
- Act as a liaison between medical providers and informal supports.
- Facilitate preventative care referrals tied to Health Action Plan goals.
- Aim to reduce emergency room visits and hospital readmissions.
- Use health technology to connect beneficiaries with services.
- Assist beneficiaries and families with empathy and confidentiality.
- Handle calls and inquiries promptly for accurate communication.
- Conduct outreach to engage eligible beneficiaries and educate partners.
- Respond to individual cases within Health Home scope, including post-hospital discharge follow-up.
- Review Health Action Plans every four months to maintain eligibility.
- Promote the Mission, Vision and Values of Transforming Age in all areas of responsibility, and demonstrate commitment to Culture of Excellence and strategic goals
- Performs other duties as assigned.
What you need to succeed:
- Bachelor’s Degree or higher in Social Work, Psychology, or related field and a minimum of two (2) years’ work experience in human services, healthcare, mental health or related field
- Must have excellent verbal and written communication skills.
- Must have access to reliable means of transportation; may require reliable vehicle for client transport.
- Current Washington State Driver’s License.
- Proficiency in MS Office and EHR software or comparable programs.
Physical Requirements include:
- Ability to lift/push/pull up to 25lbs.
