Compensation:
- In accordance with California law, the expected salary range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
Role Summary
As a Lead Care Manager, you will be pivotal in supporting children, youth, and families facing life’s toughest challenges. Whether you provide hands-on case management, help to develop life skills, or provide emotional support, your work will directly impact the lives of those we serve.
What We’re Looking For:

How you will Impact Change:
- Utilizes standardized screening tools to assess client needs in the areas of physical health, mental health, SUD, oral health, trauma-informed care, social supports, housing, vocational/employment, wellness, and referral and linkage to community-based services and supports.
- Oversees the development and implementation of the Individual Care Plan/Health Action Plan
- Offers services where the client lives, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services.
- Connects clients to other social services and needed supports (e.g., community support group).
- With permission, coordinate/advocate on behalf of the client with health care professionals (e.g., PCP).
- Utilizes evidence-based practices, such as motivational interviewing, harm reduction techniques, and trauma-informed care principles.
- Conducts outreach and engagement activities to facilitate linkage to the ECM program. Outreach and Engagement consists of phone calls, mailed information, and field visits.
