High Impact Unit (HIU) Worker I

Educational Requirements:

Bachelor’s degree in social work or a field applicable to the specified job responsibilities and two years of relevant experience

Work Experience Requirements:

Minimum of five years of experience working with transitional aged youth with a demonstrated track record of successful engagement
Experience in Foster care/Kinship care preferred

Supervisory Experience:

Supervisory experience not required but project oversite/management preferred

Job Summary:

Serve as a specialized support lead to provide services and resources to children of all ages within the High Impact Unit, including transitional living age in five key areas: 1) Workforce Development and Employment Opportunities; 2) Education and Housing Opportunities; 3) Coordinate learning and community activities to facilitate successful transition into viable independent living circumstances; 4) Assist with finding permanent lifelong connections to act as stabilizing reinforcement for the present as well as future years following exit from the child welfare system. 5) Work to find and establish extracurricular activities and outing for all aged youth based on their specific needs.

Qualifications:

Five years of direct relevant experience in areas such as workforce development, academic advising, and housing counseling preferred
Must display an ability to interact effectively with various business, education and housing constituencies
Must possess excellent oral and written communication skills, exemplify excellent organizational skills; be extremely detail-oriented; demonstrate strong interpersonal skills
Strong interpersonal skills, maturity, creativity, cultural competency, good judgment and capable of effective oral and written communication, training and presentations to diverse groups, including staff, kinship families, government officials, private sector service providers, etc
Broad functional experience in areas of strategic planning, public policy, program evaluation, outcomes, and research

Responsibilities:

Oversee assigned cases referred to the HIU department.
Responsible for tracking and document collection of youth participating in the HIU program.
Maintain HIU “One-on-One” specific caseload of 12- 15 youth, inclusive of “crossover” youth, and maintain all necessary case file paperwork.
Provide direct 1 on 1 service delivery to children participating in the HIU utilizing the “Empowerment checklist”, stabilization plan and additional assessments/plan completion.
Meet at a minimum of weekly with youth to engage in intensive relationship building and stabilization.
Engage with Point of Contact Caseworkers, CYF, Probation Officers, Mental Health Providers, Kids Voice, and all other Providers to ensure that there are wholistic service delivery and a high level of support for youth in the program.
Oversee weekly Maintenance Educational Group Sessions for children of HIU. Facilitation and/or Co-Facilitation of groups may be necessary as needed.
Oversee HIU special projects (i.e. Senior Celebration; CAMP COPES; Black College Tour, Dance for Life) as assigned.
Facilitate or Co-Facilitate Staff Training relative to HIU (i.e. HIU Overviews, HIU Cross Training, etc)
Engage as designated in relation to implementing, monitoring, tracking and assessing all service avenues offered in the HIU (i.e. Preparation for a Generation; Maintenance Group Connections; Goal Plan Requirement; Prevention through Preparation Group Connections; and Recede the Recidivism Campaign)
Strategize and drive local prospecting to establish, build and maintain relationships with local businesses, institutions, and the community to ensure employment opportunities are available to youth exiting the Youth Engagement Services Program
Assist in special project coordination relative to training activities for participants to encourage workplace ethics and promote workplace etiquette (i.e. Job Readiness Program)
Assist in special project coordination relative to educational activities for participants such as college fairs, SAT prep courses, etc. – as assigned
Assist in the development of educational curriculum and implementation of the developed curriculum which will include but not be limited to the following:
- Help develop and maintain a pool of rental/leasing, real estate options for youth
- Material relative to social services and social work programs that assist with securing housing options
Forging corporate, community and institutional partnerships in order to assist in meeting and expanding the objectives of the HIU
Keep accurate records and statistical data in cooperation with the department Director
Prepare and submit reports as requested
Serve in an on-call rotation to support HIU youth and youth in crisis from 12 to 21 years old
Other duties as assigned

Requirements:

Registered and insured vehicle
Certificate of Insurance for vehicle
Valid Pennsylvania Driver’s License
City/County travel and some long-distance travel
Current Act 33 child abuse and Act 34 Police Clearances, as well as FBI fingerprint check
Available to work a flexible schedule, as needed including evenings and weekends.
Must carry an agency cell phone.
Ability to travel to the agency’s Philadelphia Office and other needs on occasion is required.
