What are we looking for?
We seek a highly flexible and reliable Floater Direct Support Professional (DSP) who can adapt quickly to various assignments and locations as needed. This role requires someone proactive, dependable, and comfortable stepping into different environments to provide quality support. The Floater DSP must be able to learn routines quickly, follow individualized support plans, and collaborate effectively with various teams. Strong problem-solving skills, the ability to work independently, and a commitment to person-centered care are essential.
Your overall essential responsibilities will be the following:
Medical Appointment Coordination
- Transport individuals to and from medical, dental, and therapy appointments safely and on time.
- Serve as the primary point of contact during appointments, advocating for the individual and ensuring their needs are addressed.
- Communicate effectively with healthcare providers to gather and understand diagnoses, treatment plans, and instructions.
Documentation & Communication
- Document all appointment details, including diagnoses, treatment plans, and follow-up recommendations, in Therap or other designated systems within required timeframes.
- Ensure accurate and comprehensive health-related updates are shared with the individual support team, including Residential Team Leaders, Program Managers, and family members as needed.
- Maintain confidentiality and compliance with HIPAA regulations when handling medical information.
Follow-Up Care
- Monitor and coordinate follow-up appointments, referrals, and prescriptions to ensure continuity of care.
- Track and manage medical records, lab results, and other health-related documentation.
- Collaborate with the team to ensure that prescribed treatments, dietary restrictions, and medical equipment needs are implemented and followed.
Individualized Support
- Build a rapport with individuals to reduce anxiety related to medical appointments.
- Demonstrate patience, empathy, and understanding of the unique needs of individuals with IDD.
- Encourage and support self-advocacy for individuals who can actively participate in their healthcare decisions.
Problem-Solving
- Address and resolve scheduling conflicts, transportation challenges, and unforeseen health-related issues.
- Proactively identify barriers to accessing care and recommend solutions.
Other Duties
- Assist with other DSP responsibilities as needed, including community outings, daily living skills, and personal care.
- Participate in training and professional development activities to stay informed about best practices in healthcare coordination and IDD support.

The hiring range for this position ranges from $17.50 - $18.50, depending on experience.
