SUMMARY
Counsels consumers and their families and provides assistance in understanding illness and disabilities and helps resolve personal, financial and environment difficulties that interfere with obtaining maximum benefits. Works in accordance with ADAPT Community Network (ADAPT) policies, the rules and requirements of applicable regulatory agencies and the standards of the profession. Develops collaborative relationships with other disciplines to assure proper client care.
PART TIME - Flexible 30 hours per week.
COMPENSATION : $30.00 - $40.00/hourly plus industry leading benefits
ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Social Worker will include, but are not limited to the following: • Participates in intake process and develops initial and annual social service summary on each person receiving services. • Develops a treatment plan and goals based on information in summary. • Provides social services as prescribed by treatment plan in areas of counseling and concrete services including housing and residential placement, food stamps, Medicaid, half-fare cards, camp, vacation referrals, etc. • Integrates therapeutic intervention techniques into programs or home of person served through consultation with parent/guardian and staff at meetings, case conferences or through other informal contacts. • Participates in interdisciplinary team meetings and annual reviews as required. • Records progress on all active cases, monitors status and makes appropriate recommendations as changes occur. • Assists with arrival, dismissal, feeding, toileting as needed. • Directs clients/families to appropriate community agencies and resources for financial assistance, counseling, auxiliary care, adaptive devices, etc. • Keeps abreast of changes in field. • Safeguards information in relationship to individuals receiving services, including clinical, program and family information. • Trains and instructs staff in utilization of goals, rights of persons receiving services, confidentiality, family interaction, etc. • Provides supervision and training for Social Work Assistants and less experienced Social Workers. • Makes regular home visits to families on case load. • Attends professional meetings and conferences. • Performs other related duties as requested.

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Ability to read, write, speak and understand English • Good interpersonal skills are required in order to interact effectively with employees, coworkers, clients, families and outside contacts. • Good organizational skills • Proficiency in the use of a personal computer and knowledge of appropriate software • Due to sensitive nature of the position, must be able to maintain the strictest confidentiality. • Ability to work independently and handle multiple tasks. • Excellent organizational skills • Able to work flexible hours • Demonstrate sound judgment and the capacity to respond to unusual situations. • Good oral and written communications skills
EDUCATION and/or EXPERIENCE
• Master’s Degree in Social Work from an accredited program
• One year of prior experience as a Social Worker preferred.
LICENSE/CERTIFICATION
• New York State Certification (CSW) NOTE: Individuals with New York State Certification in Social Work (CSW) have certification that was automatically converted to the New York State Licensed Master Social Worker (LMSW).
