JOB SUMMARY: This position is responsible for the coordination and delivery of addiction treatment services designed to meet the recovery and wellness needs of clients within the For My Baby & Me and addiction treatment programs. Depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumer's family members.
Under the supervision of the RN care manager, the Substance Use Screener will arrange drug testing for clients referred to the For Baby & Me Program. The navigator will follow all the established protocols for the program as detailed in program description. The staff member will ensure all documentation procedures are done in accordance with agency policies and procedures, ensuring appropriate assessment tools are utilized, recorded, and that the appropriate treatment referral is made.
ESSENTIAL FUNCTIONS
Services:
- Conduct initial For My Baby and Me program screening and ongoing assessments involving the collection and recording of significant bio-psychosocial information as needed.
- Complete evidence-based perinatal screenings to help conceptualize the interrelatedness of diversity, culture, disease course, and the larger family system.
- Supports clients' strengths and resources, affirming reality testing, contact with experience, judgment and connection with support systems including family, friends, and community resources, including assisting with housing- connecting to local resources including domestic violence.
- Partner with the client and their support persons (if necessary) to identify goals, increase engagement in their healthcare, address barriers, and drive the execution of their care plan.
- Proactively support our highest-need clients by providing comprehensive assessments, telephonic check-ins, and in-person visits.
- Maintain extensive knowledge of community resources and make referrals, as appropriate, to help ensure the client’s healthcare and psychosocial needs are met.
- Navigate client insurance benefits to ensure mindful utilization and maximization of plan benefits.
- Provide both longitudinal and episodic comprehensive care coordination with the goal of empowering clients to independently manage their health conditions and have increased quality of life.
- Educate clients on supportive care options and provide goals of care counseling, support, and advocacy at all stages of the patient journey.
- Provide psychoeducation one on one or group to clients and families surrounding life stage and perinatal issues, caregiver support, etc.
- Meet with care teams to collaborate and generate creative solutions to client issues.
- Complete concise and comprehensive documentation on time to ensure that all care team members have visibility into the status of clinical interventions based on licensure.
- Partner with Community Health Workers and provide clinical guidance and consultation regarding complex client needs to these team members.
- Respond to crises and urgent needs with the keen ability to prioritize work, triage situations, and multitask.
- Consults with other service providers and documents their participation in the recovery process.
- Maintains established program productivity and data requirements.
Documentation:
- Enters reports and assessments into information technology system as indicated; works with collaborative agencies to ensure data measures are captured and met.
- Maintains accurate and up-to-date case records in accordance with agency standards, accreditation, and funding source requirements.
- Assures that all HIPAA standards are followed with regards to client information

- Ability to demonstrate knowledge of the dangers of substance use during pregnancy
- Ability to accurately maintain electronic medical records.
